Your information stays with you. What you type is used only inside this page to build the card. It is not sent to us or anyone else. If you tick “Keep on this device”, it is saved in this browser’s storage so you can update it later; otherwise it disappears when you close the page. Do not enter Aadhaar, bank, card or policy numbers: a lost card should not expose them.

Tips for a card that helps
- Write allergies even if there are none. “None known” tells a doctor something; a blank tells them nothing.
- Use plain words for conditions: “diabetes, takes insulin”, “had a stroke in 2023”, “pacemaker”.
- Medicines: list them, or write “see medicine chart” and keep the printed chart with the card.
- Two contacts who answer their phones, ideally one living nearby.
- Emergency numbers: write 112, India’s national emergency number, and any local ambulance or hospital number the family uses.
- Date it, and check it whenever a medicine, doctor or phone number changes.
- Where to keep copies: in the wallet or handbag, on the fridge door, in the hospital document folder, and a photo on a family member’s phone.
These tips are our own practical suggestions. The details about 112 come from the Government source below.
